Student Health Services Factsheet

Welcome Letter

Dear Students,

Welcome to Student Health Services. We are a dedicated group of professionals including physicians, nurse practitioners, nurses, medical assistants, administrators and other medical personnel available to take care of your health care needs at The Claremont Colleges.

In an effort to better serve you please review the SHS website for more details about our services and programs. Many of the forms and procedural information is detailed on our website. Please be sure to thoroughly read the New Student Guide to SHS before arriving on campus. The entrance physical form for undergraduates  and graduate students must be completed and submitted to SHS. These forms also list the required immunizations. Completed forms can be submitted to SHS’s Medical Record Uploader here.

Warmly,
Prateek Jindal, DO
Director – Student Health Services
