Student Health Services

Student Health Services provides patient-centered health care to our campus community in a professional and compassionate manner that directly contributes to the student’s well-being and overall success. Student Health Services at The Claremont Colleges is a service of The Claremont Colleges Services and is provided to all undergraduate students enrolled at The Claremont Colleges.

Scheduling Appointments

All registered students attending The Claremont Colleges are eligible to be seen at Student Health Services. Our staff of physicians, nurse practitioners, and nurses makes every effort to provide high quality medical services in a caring and efficient manner.

All services are private and confidential. Any information regarding these services is not released to anyone without the student’s signed permission. In the interest of providing comprehensive wellness care, Student Health Services primary care providers and Monsour therapists may consult with one another when needed.

To schedule an appointment, sign into My Health Portal  or call (909) 621-8222. The appointment line opens at 8 a.m. and appointments are scheduled starting at 8:20 a.m. Same-day appointments are often available if you call early.

In the event of a medical emergency please, call Campus Safety at (909) 607-2000 (on-campus extension x72000) and describe the nature of the emergency. If you are off campus please call 911.

In the event of a mental health emergency, please call (909) 621-8202 and dial “1” to be connected to an on-call therapist.

Appointments scheduled in advance are highly recommended for all visits and are free to students.
Non-scheduled visits will be charged $15 and may be subject to significant wait times before meeting with a provider. Unscheduled visits after 4:30 p.m. may be scheduled for the following day or sent to Urgent Care.
Please note that some services may require additional costs (X-ray, laboratory, medication, etc.). Students have the right to decline or refuse services.
A $15 no-show charge is assessed for missing an appointment or canceling without two hours advance notice.
Appointment reminders are emailed to students prior to a scheduled appointment. If you would like to opt out of receiving the reminder email, please notify the front desk staff

This form is required by Student Health Services. All students must have a completed form on file. This form must be completed by a healthcare provider and signed by the student or a guardian if the student is under the age of 18.

  • Student Health Services strictly adheres to all state and federal regulations pertaining to confidentiality of medical records.
  • A release of information form must be completed when a student makes any request for their patient information to be released to parent, family practitioner, referral doctors or anyone else.
  • This form or a written request (must include DOB/School attended/specific information requested and where to send or Fax information/signed and dated) must be completed by an enrolled student requesting copies of his/her medical file.
  • Copying charges may be applied and billed to your student account.
  • The completed release form or written request may be mailed, faxed or hand delivered to Student Health Services and the request will be handled as promptly as possible.
  • Fax requests for records by another physician must be accompanied by a signed request for release of medical records and will be sent at no charge, fax: (909) 621-8472.
  • We do our very best to see that all requests are addressed promptly. However, during the summer break (June – August) it may take up to 15 business days to provide the information requested.
