Entrance Health Requirements are due by July 15 th . If you are unable to submit your completed requirements by July 15, please do so prior to your arrival on campus. If you are unable to complete your physical or entrance immunizations, please contact SHS at (909) 621-8222 before your arrival on campus to schedule an appointment.
