What can I expect for a sports physical?

Sports physicals are available by appointment at SHS. Sports physicals are generally not available same-day so we recommend scheduling well in advance of the due date. Sports physicals generally take about an hour. The cost for sports physicals is $20. Please bring the sports physical form (can you link the attached document here?) with the patient section completed and arrive to SHS 10 minutes prior to your scheduled appointment time.

What can I expect for a study abroad physical?

Study abroad physicals are available by appointment at SHS. Study abroad physicals are generally not available same-day so we recommend scheduling well in advance of the due date of the forms. When you schedule a study abroad physical, we will ask what country or countries you plan to study in or travel to during your time abroad. What can I expect with STI testing?

When you request a visit for STI testing, we will ask if you are requesting routine STI testing, symptomatic STI testing, or if you have been notified of exposure by a partner. If you are requesting routine STI testing, we will request that you hold your urine for about an hour prior to your appointment and to make sure you are well hydrated. I am a CGU/KGI student that was previously seen at SHS or MCAPS. How do I obtain my records to transfer care to another provider?

Students needing to transfer care can get their medical records to submit to their new provider. You can obtain your SHS records using the Health Portal: https://shshealthportal.cuc.claremont.edu/confirm.aspx.   MCAPS records are available by request. Please review the MCAPS website at https://services.claremont.edu/mcaps/faqs/ for answers to frequently asked questions, mental health resources, and information on obtaining your medical records. Please note that MCAPS medical records transfer will be unavailable from May 30th through August 1st, annually.

Can I schedule a brief assessment if I live out of state or out of the country?

Yes, it is fine to schedule a brief assessment if you currently reside outside the state of California, including outside the U.S. The MCAPS therapist will consult with you about what type of mental health support you are seeking and will help you find appropriate care in your local area.

Do I need to schedule a brief initial assessment if I am only seeking psychiatry (medication) services?

No. If you are sure that medication management is the only mental health support you are looking for, it’s OK to schedule a one-hour initial medication evaluation directly with one of the MCAPS psychiatrists. If you are seeking continued medication services with a psychiatrist you have previously worked with, you can schedule a 30-minute ongoing medication check appointment.
